The
Rathfredagh Cheshire Service aims:
- To
continue to deliver its existing service to the
residential population
- To
respond to the emerging needs of persons with
physical and sensory disabilities in the
community through day and respite services.
- To
enhance the lives of persons with physical and
sensory disabilities through social interaction
in the community and through Internet access.
Reflection
on Objectives 2001
- All
Cheshire services aim to continue and develop the
social model of care within our services. To do
so requires that all practices be evaluated
against this model and practices changed where
neccessary.
- The
service reached new population in 2000. However
we acknowledge that we do not always reach the
young population of disabled people and a stategy
to do this will be developed in 2001.
Service
Integration
- Case
conferenceing involvement with the MWHB and other
voluntary service providers to develop packages
of care for individuals and their families
- Develop
networks with statutory and voluntary housing
providers to provide community housing with
asisted living support to persons with physical
and sensory disabilities.
- Planned
respite provision to maximise respite systems for
families.
- Ongoing
service development needs identifications through
Mid Western Health Board Co-Ordinating Committee
and other organisations.
Performance
Review
- Occupancy
levels for all services to be targeted 100%
- Reduction
of waiting list through exploration of
alternative services for clients.
- Implementation
of integrated care plans for service users and
achievement of target outcomes as identified by
clients and staff.
- Implementation
of planned capital expenditure.
- Enhance
service integration across statutory and other
voluntary organisations through partnership.
